Stacked PRs have `pullId: 0` in PDS #135

I'm not sure how stacked PRs are handled, but this seems to be a bug.

agreed, i believe there is a bug in newStack in appview/pulls/pulls.go, i will take a look at this.

for what its worth, pullId should be removed entirely from the record, it is something that is appview specific.

pullId has been removed from the lexicon now.
